What actually causes it

Orientation. Stored mouthpiece-down, gravity works against the wick and liquid pools where the airflow exits.
Very hard draws. Pulling harder than the airflow is designed for drags liquid past the wick rather than vaporising it. This also causes gurgling.

Cleaning it up

Wipe the outside and the mouthpiece with a dry tissue, then invert the device and draw two or three times without firing to clear liquid from the airway. Wash your hands afterwards — nicotine is absorbed through skin, and a concentrated e-liquid is not something to leave on your fingers.

Keep leaked liquid away from children and pets, and wipe down anything it pooled on.

Preventing it

Store it upright with the mouthpiece up. Keep it out of hot cars and away from radiators and windowsills. Draw gently rather than hard. If you carry it in a bag, a small zip pouch contains any leak that does happen and costs nothing.

Gurgling is the same problem, earlier

A gurgling or spitting sound means liquid has already collected in the airway. Invert and draw a few times without firing to clear it before it becomes a leak.

When it is a defect
